Case Study: High-Risk ICU Transfer from Hatyai Hospital to Johor
Recently, we received an urgent call for assistance from Hatyai Hospital in Thailand. A patient had suffered a post-cardiac arrest and required immediate transport to Hospital Sultanah Aminah in Johor, Malaysia.
The patient’s condition was highly critical:
- Mechanically ventilated
- Dependent on triple inotropes
- Receiving multiple intravenous medications to stabilize heart rhythm
Due to a lack of available ICU beds in Hatyai, the family requested to transfer the patient back to Malaysia to be closer to home and secure definitive critical care. After the referring medical team explained the extreme risks of moving a patient in this condition, the family signed a discharge against medical advice to proceed with the transfer.
The Emergency Response and Cross-Border Journey
Upon confirmation, Medcall dispatched a specialized medical team comprising an Intensivist and a Flight Paramedic. Simultaneously, our medical coordinator liaised directly with the receiving specialists in Johor to ensure a seamless handover. Anticipating a high-risk journey with the potential for further cardiac events, the team prepared a comprehensive suite of medications, ranging from sedatives to advanced blood pressure stabilizers.
Because the patient had suffered a cardiac arrest less than 48 hours prior, Targeted Temperature Management (TTM) was initiated and carefully maintained inside the ambulance throughout the journey from Thailand.
During the long transit, the patient suffered two separate episodes of cardiac arrest. Thanks to the mobile ICU setup and the expertise of our on-board specialists, the patient was immediately defibrillated, and a pulse was successfully restored both times. Intravenous arrhythmia medications were administered to restabilize the patient en route.
Successful Arrival and Ongoing Care
Despite the severe complications during transit, the patient was successfully conveyed to Hospital Sultanah Aminah and immediately underwent surgery. While his condition remains critical, he is currently recovering in the ICU. The entire Medcall team continues to pray for his speedy recovery.
